DV360 dynamic creative keeps showing the same 3 variations out of 40, rotation isn't working

Feed is populating correctly, all 40 combinations pass the preview check, but live delivery keeps cycling the same three. Optimization is set to even rotation, not performance-weighted, so it shouldn't be favoring anything yet. Anyone run into DCO rotation just getting stuck like this?
